Studio City Dental Group has partnered with a variety of quality health care
financing companies as well as developed some private financing options. If you
have additional questions about the financing options available to you, please
give us a call so we can provide the information you need in order to make the
best decision.

To apply for financing options, please follow these instructions carefully.
- Click on the link at the bottom of these instructions
- Input "Studio City Dental Group" in the search box in the new window that opens
up
- Select either of the 2 "Studio City Dental Group" options
- Click the "YES" button and then follow the remaining instructions until the
application is complete

New Patient Forms
If you are a new patient, click on the new patient form below. Print the form and
fax it back prior to your appontiment to spend less time in the waiting room.
You may also have your most recent x-rays emailed to us before your appointment date. Email them to our office, include your full name and the date the x-rays were taken, at: info@studiocitydentalgroup.com
